I submitted the kicad_pcb file to OSH Park for creating a 2 layer PCB. This board is a more compact than my first attempt and the inputs and outputs are in the same order (Red, Yellow, Green).

I got the email back "Your boards are placed in a queue awaiting panelization. From there they will be assigned to panels on a first-come-first-served basis. You'll get another email notification when this happens. This shouldn't take more than a couple of days"
Hopefully I'll get the board delivered around October 11th. No idea if I messed anything up, so I guess I just have to wait a week or two. If there are errors, then I'll make slight iterations on this current design (e.g. v1.1)
For version 2.0, I am thinking of adding pullup resistors on the inputs, LEDs on the outputs, and make the 4023 utilize surface mount technology.
